Advance trained pharmacists provide comprehensive medication management in a collaborative environment with the patient and their care teams. Each pharmacist has advanced training with experience in pain syndromes, and sees patients one on one. Patients receive answers to their medication questions, participate in their own medication care-planning, and get assistance in ensuring they are able to manage their therapy at home for the best possible outcomes. The clinic models the best practices about client-cantered care, informed decision-making, virtual collaboration in care teams, and interpretation of the evidence.
The Pharmacists Clinic at the UBC Faculty of Pharmaceutical Sciences is Canada's first university-affiliated, licensed, pharmacist-led patient care clinic. It provides the highest possible standard of health consultation services to patients, learning opportunities for health professionals and students alike, and a living laboratory environment that supports health care research and evaluation.
